Job summary

Graduate Post Acute in Philadelphia seeks a compassionate RN to deliver skilled nursing care in a 150-bed sub-acute and long-term care environment. You will monitor residents, administer medications, and collaborate with an interdisciplinary team to uphold care plans and regulatory standards.

Starting pay is $37–$46 per hour, all-inclusive, with opportunities for full-time, part-time, and PRN positions. Benefits include tuition reimbursement, 401(k) with match, shift differentials, and more.

Qualifications

  • Active RN license in Pennsylvania.
  • Valid BLS Certification (CPR) required.
  • Ability to manage care for multiple residents in a fast-paced setting.
  • Strong clinical assessment, communication, and documentation skills.

Responsibilities

  • Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
  • Follow and implement care plans developed by the Nurse Manager.
  • Perform medication administration and treatments in accordance with the plan of care (POC).
  • Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
  • Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
  • Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
  • Ensure compliance with infection control protocols, safety standards, and nursing regulations.

  • Strong clinical assessment, communication, and documentation skills.
