Encompass Health Rehabilitation Hospital of Toledo is seeking an Admissions Liaison whose work brings patients closer to home and cares deeply about their admission journey. You will coordinate admissions, handle referrals, verify information, and facilitate scheduling for both inpatients and outpatients.

This role emphasizes compassionate service and efficient processes within a home-like hospital environment.

Qualifications

• One or more years in hospital admissions procedures, preferred.
• Licensure as a clinician preferred.
• Current CPR certification preferred.
• CRRN preferred.
• A strong understanding of multidisciplinary care levels and at least one year of clinical experience are advantageous.

Responsibilities

• Coordinate an efficient admissions process for all patients.
• Preform pre-certifications.
• Maintain accurate listing of all patients in hospital.
• Receive and verify referrals, gather essential information and execute necessary procedures for scheduling and admission.
• Contribute to the referral process and input statistical data into hospital systems.
