Goldwater Care is seeking an Admissions Liaison to be the first point of contact for prospective residents, families, and referral sources. You will guide inquiry, intake, and admission while coordinating with hospitals and facilities regarding requirements and placement.

You will gather and verify essential demographic, financial, medical, and referral information to support accurate, timely admissions and ensure confidentiality and professional service across all interactions.

Qualifikationen

• Must have 2+ years in a healthcare setting.
• Excellent written and spoken communication.
• Strong organizational and time-management skills.
• Ability to handle sensitive information with discretion.

As an Admissions Liaison at Goldwater Care, you will serve as a primary point of contact for prospective residents, families, representatives, referral sources, and community partners, providing a welcoming and professional experience throughout the inquiry, assessment, intake, and admission process.

Essential Duties and Responsibilities:

• Serve as a primary point of contact for prospective residents, residents, families, representatives, referral sources, and community partners, providing a welcoming and professional experience throughout the inquiry, assessment, intake, and admission process.
• Conduct bedside assessments and referrals at hospitals, healthcare facilities, and other locations in collaboration with prospective residents, representatives, families, case managers, social workers, and other healthcare professionals to evaluate appropriateness for admission.
• Gather and verify necessary demographic, financial, medical, and referral information to support accurate, timely, and compliant admissions.
• Coordinate and facilitate the admission process, including communicating with prospective residents, families, referral sources, and facility staff regarding admission requirements, placement, and available services.

Requirements:

• Must possess a minimum of 2 years experience in a healthcare setting.
• Ability to communicate effectively, orally and in writing.
• Strong organizational and time management skills.
• Ability to maintain confidentiality and handle sensitive information.
