Responsibilities

• Lead the admissions process, coordinating patient intake, registration, and bed placement
• Collaborate with care teams to implement admissions strategies focused on patient-centered care
• Develop and maintain relationships with referring hospitals, physicians, and healthcare partners to increase referrals
• Conduct patient assessments and develop care plans with the care team
• Participate in quality improvement initiatives to enhance patient experience and outcomes
