The role focuses on developing referral networks, coordinating admissions from referral to admission, and ensuring smooth transitions for residents while meeting census goals.

Key Responsibilities

• Proficiency with Microsoft Office and electronic medical record systems.
• Valid driver's license and reliable transportation for local travel.
