Job summary

Complete Care at Willow Creek is seeking an Admissions Director to lead the admissions process, coordinate with hospitals, and develop partnerships with referral sources. You will oversee inquiries, discharges, and census reporting while ensuring a smooth transition for patients and families.

Qualifications

  • Previous admissions experience in a skilled nursing or long-term care setting preferred.

  • Ability to work independently and collaboratively.
  • Basic computer proficiency.

Responsibilities

  • Oversee inquiries, admissions, and discharges while supporting census reporting.
  • Coordinate the admissions process with documentation completed accurately and on time.
  • Communicate with nursing, business office, and external referral sources to coordinate admissions and bed availability.
  • Build and maintain relationships with hospitals, case managers, and referral partners to support census growth.
  • Evaluate referrals and assist with admission decisions.
  • Maintain accurate census, referral, admission, and discharge data.
  • Monitor admissions activity, referral trends, and discharge planning with reports as requested.
  • Deliver exceptional customer service to residents, families, and referral sources throughout the admissions process.
