Job summary

Complete Care is seeking an Admissions Director to guide patients and families through transition into our skilled nursing and rehabilitation center. You will coordinate admissions with hospitals, referral partners, and the community, ensuring a smooth census process and exceptional service.

The role emphasizes strong communication, relationship-building, and teamwork within a warm, patient-focused environment at our newly renovated facility.

Qualifications

• High school diploma or equivalent required.
• Previous admissions experience in a skilled nursing or long-term care setting preferred.
• Positive, energetic, and customer-focused attitude.
• Strong communication and relationship-building skills.
• Ability to work independently and collaboratively.
• Basic computer proficiency.

Responsibilities

• Oversee admissions, inquiries, and discharges while supporting accurate census reporting.
• Coordinate the entire admissions process, ensuring all required documentation is completed accurately and on time.
• Communicate with nursing, the business office, and external referral sources to coordinate admissions and bed availability.
• Build and maintain relationships with hospitals, case managers, and referral partners to support census growth.
• Evaluate referrals and assist with admission decisions.
• Maintain accurate census, referral, admission, and discharge data.
• Monitor admissions activity, referral trends, and discharge planning while providing reports as requested.
• Deliver exceptional customer service to residents, families, and referral sources throughout the admissions process.
