Harmony Health and Rehabilitation in Fitzgerald, GA is seeking an Admissions Director to lead referrals, tours, and the full admission process from first contact to move-in day. You’ll build relationships with hospitals and community partners, verify insurance, coordinate with nursing and admin teams, and ensure a smooth, compassionate transition for residents and families.

Formación

• Experience in SNF admissions or case management.
• Working knowledge of Medicare, Medicaid, and managed care.
• Excellent communication and customer service.
• Organized, detail oriented, and able to work in a fast-paced environment.
• Compassionate approach with residents and families.

Responsabilidades

• Manage incoming referrals from hospitals, physicians, and community partners
• Review clinical info and verify insurance and payer eligibility
• Give tours and walk families through the admission process
• Coordinate admissions with nursing, therapy, social services, and business office
• Complete admission paperwork accurately and on time
• Build and maintain relationships with hospital case managers and referral sources
• Track census and referral activity and report to leadership
