Job summary

Sanford Health is seeking an Admissions Coordinator to identify potential admissions and manage the admissions process for residents and clients. The role involves processing referrals, verifying insurance, authorizations, and coordinating move-in logistics.

Responsibilities include working with the interdisciplinary team to ensure appropriate medical services, conducting clinical evaluations, and maintaining compliant documentation across CRM and EMR systems.

Qualifications

• Bachelor's degree from an accredited school and/or program is preferred, but not required.
• Currently holds an unencumbered license in RN, LPN, OT, PT or LSW with applicable State Board and/or multi-state licensure.
• Experience in long term care facility preferred.

Responsibilities

• Identify potential admissions and process new resident/client requests.
• Coordinate resident move-in process and admission paperwork.
• Perform clinical evaluations on resident/client charts to determine services.
• Collaborate with referral sources to plan and evaluate admissions process.
• Educate healthcare team on trends, regulations, and policies affecting admissions.
