The Laurels of Chagrin Falls is seeking an Admission Coordinator to support daily admissions operations and assist patients transitioning from hospital or community settings. You will review medical, financial, and other records with department heads to ensure suitability for the facility.

Responsibilities include referrals, bedhold calls, pre-admission coordination, tours, screenings, and documentation. A 4-year degree is preferred and familiarity with AI-driven software is a plus.

Qualifications

• High School diploma required; a 4-year degree preferred.
• Experience in healthcare, sales/marketing/insurance is a plus.
• Familiar with medical terminology and AI-driven software.

Responsibilities

• Receives resident referrals via fax, phone, or internet and follows admissions process.
• Completes bedhold calls on all hospitalized guests.
• Manages 30-day Medicare postcard mailings and maintains recipient database.
• Conducts facility tours for potential residents and families.
• Performs admission screening to determine level of care and services needed.
• Coordinates admissions with staff and obtains approvals.
• Ensures OBRA PASSAR/MI-MR approvals when indicated.
• Obtains payer authorization numbers per case when applicable.
• Coordinates pre-admission services with clinical staff.
• Welcomes residents and families on admission; collects necessary docs.
• Coordinates room assignments with Nursing and tracks census reports.
• Documents pre-admission referrals activity in PCC CRM.
