Job summary

Momentum at South Bay for Rehabilitation and Nursing in East Islip, NY is seeking a Full-time Admissions Coordinator. You will conduct tours, admit residents, and explain admission documents to applicants, hospital discharge planners, and community agencies.

This in-person role requires strong communication, interview and assessment skills, and knowledge of entitlements and elder care resources. The position includes health, dental and vision insurance, 401K and PTO.

DUTIES AND RESPONSIBILITIES:

• Conduct tours and provide information about Momentum at South Bay to applicants, hospital discharge planners and community agencies.


• Admit residents in accordance with established policies and procedures.


• Upon admission, responsible to complete, review and explain admission documents to residents and/or sponsor obtaining signatures as necessary.


• Complete pre-admissions in Sigma EMR.


• Other related duties as assigned by Admissions Director
