Job summary

Creekside Post-Acute is seeking an Admissions Coordinator to manage the end-to-end admission process for new residents in our skilled nursing and post-acute setting.

You will coordinate with hospitals, families, and referral sources, verify demographics and insurance, and ensure compliance with HIPAA while maintaining exceptional customer service.

Qualifications

  • Previous experience in Admissions, Intake, Patient Services, or a similar healthcare role is preferred.
  • Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, or healthcare setting is strongly preferred.
  • Familiarity with Insurance verification and authorization processes is ideal.
  • Knowledge of Electronic Medical Records (EMR) and healthcare information systems is beneficial.
  • Excellent verbal and written communication skills are essential.
  • Strong organizational skills and attention to detail are required.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Professional and compassionate approach when working with residents and families is important.

Responsibilities

  • Coordinate and oversee the admission process for new residents from referral through admission.
  • Collect, review, and verify patient demographic, medical, and insurance information.
  • Verify insurance eligibility, benefits, and coverage requirements.
  • Communicate with hospitals, physicians, referral sources, residents, and family members regarding admissions.
  • Coordinate with nursing and other departments to ensure preparation for each new admission.
  • Maintain accurate admission records and documentation.
  • Ensure required paperwork is obtained prior to and during admission.
  • Respond to questions from residents and families in a professional manner.
  • Maintain timely communication with referral sources regarding admission status.

Position Overview

TheAdmissions Coordinator is responsible for coordinating and overseeing the admission process for new residents entering our skilled nursing and post-acute care setting.

This position serves as an important connection between prospective residents, families, referral sources, insurance providers, and our internal clinical and administrative teams. The ideal candidate is highly organized, professional, responsive, and comfortable managing sensitive patient information while maintaining a high level of customer service.

Key Responsibilities
  • Coordinate and manage the admission process for new residents from referral through admission.
  • Collect, review, and verify patient demographic, medical, and insurance information.
  • Verify insurance eligibility, benefits, and coverage requirements.
  • Communicate with hospitals, physicians, referral sources, residents, and family members regarding admissions.
  • Coordinate with nursing and other departments to ensure appropriate preparation for each new admission.
  • Maintain accurate and complete admission records and documentation.
  • Ensure required paperwork and information are obtained prior to and during the admission process.
  • Respond to questions and concerns from residents and families in a professional and compassionate manner.
  • Maintain timely communication with referral sources regarding admission status and requirements.
  • Work closely with the interdisciplinary team to support a smooth transition into the facility.
  • Maintain confidentiality and comply with HIPAA and applicable healthcare regulations and facility policies.
  • Assist with other admissions-related and administrative responsibilities as needed.
What We're Looking For
