Job summary

City-View-Multi-Care-Center in Cicero, IL seeks an Admissions Coordinator to manage referrals, admissions paperwork, and coordination with hospitals for smooth transitions. The role centers on timely processing, payer verification, and ensuring all documentation complies with policy before admission.

You will build strong relationships with hospitals, physicians, social workers, discharge planners and other referral sources, while promoting the facility and maintaining proactive communication

Qualifications

  • Previous experience in Long Term Care, preferably in admissions and/or marketing.
  • Bachelor’s Degree preferred.
  • Must be able to read, write, and speak the English language in an understandable manner.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with residents, family members, visitors, personnel, government agencies/personnel and the general public.
  • Must possess leadership and supervisory ability and willingness to work harmoniously with all personnel.
  • Must have patience, tact, cheerful disposition and enthusiasm.
  • Must be willing to travel.

Responsibilities

  • Answer referrals in a timely manner.
  • Work with physicians and hospital discharge planners to arrange a smooth transition to the facility.
  • Market the facility to referral sources to gain new business.
  • Build and maintain relationships with hospitals, physicians, social workers, discharge planners and other referral source.
  • Complete all admission paperwork for new admissions in a timely manner.
  • Ensure facility has all required paperwork for admission.
  • Verify payer sources and ensure referral is clinically approved prior to admission.
  • Receive and coordinate investigation/follow through of concerns to the Admissions Director/Administrator any complaints received and communicate.
  • Communicate effectively and consistently with the Director of Admissions.
  • Ensure resident, visitor and employee concerns are resolved in a timely manner and communicated to person who originated concern.
  • Promote good public relations within the community.
  • Obtain copies of insurance cards and OBRA screens for every admission.
  • Complete Passar requests as needed.
  • Background checks completed on all potential admissions.
  • Perform other related duties as assigned.
  • Preparing room for new admissions.
  • Greeting and following up with new admissions.
  • Census followup in morning meeting.
  • Filing paperwork timely and organized.
  • Preparing Charts for Medical Records request.
  • Uploading Admission documents in PCC.
  • Knowledge of Computer System.
  • Customer Service.
  • Other duties per Admission Director and Director of Nursing.

Monday - Friday 9am to 5pm, MOD, weekends as needed

QUALIFICATIONS
