Job summary

Wingspan Care Group in Lorain, OH, is seeking an Admissions Coordinator to oversee Bellefaire JCB CME Intake operations and connect referrals to Wingspan’s CME services through the OhioRISE Initiative. The role reports to the Director of Lorain CME and focuses on smooth referral processing, care coordination, and interagency collaboration.

Responsibilities

  • Create a welcoming, professional atmosphere in the office.
  • Maintain client files per licensing and accreditation rules.
  • Outreach to clients/families to engage in Care Coordination.
  • Document all communications in the client chart.
  • Schedule caseload and manage high priority cases.
  • Represent the Agency with partner organizations.

QUALIFICATIONS:
  • Education: Bachelor’s Degree preferred. Minimum High School Diploma with a minimum of 3 years’ experience required.

JOB SUMMARY:

The Admissions Coordinator is responsible for the day-to-day operations of Bellefaire JCB's CME Intake Department, which ensures the all referrals for Care Coordination through the OhioRISE Initiative are connected to services with Wingspan’s CME. The Admissions Coordinator is supervised by the Director of Lorain CME.

JOB SUMMARY:

The Admissions Coordinator is responsible for the day-to-day operations of Bellefaire JCB's CME Intake Department, which ensures the all referrals for Care Coordination through the OhioRISE Initiative are connected to services with Wingspan’s CME. The Admissions Coordinator is supervised by the Director of Lorain CME.

ESSENTIAL DUTIES:
  • Create a warm, welcoming, helpful and professional atmosphere in the office for all clients and staff.
  • Responsible for creating and maintaining client files in accordance with licensing and accreditation rules. This includes opening new clients in the electronic health record and updating the system when changes of status occur to ensure consistency of database information in relation to individual client record.
  • Responsible for initial outreach to clients/families to engage in Care Coordination Services.
  • Document in client chart all communication and outreach efforts to engage in services.
  • Assure that standards/regulations set by accrediting and/or licensing bodies are maintained by Intake staff.
  • Assign/Schedule caseload to Intake Department staff and maintain a case load of high priority/special circumstances cases.
  • Represent the Agency in the development or maintenance of relationships with other organizations including sister agencies, public agencies, managed care companies, insurance companies, funders and referral sources.
  • Responsible special projects as assigned.
OTHER DUTIES:
  • Attend scheduled staff meetings, supervision, and on-going training.
  • All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  • Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
  • Respect the privacy of clients and hold in confidence all information obtained during the client's treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  • Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  • Other duties as assigned by management.
