Job summary

Socket.dev in Minnesota seeks a detail-oriented Admissions Coordinator to assist with residential and crisis services admissions and coordinate the intake process. You will manage referrals and waitlists in SharePoint, and maintain client records in Therap and Teams.

Reporting to the Admissions Director, this hourly, non-exempt role supports the Services Team with organized intake workflows, accurate documentation, and timely communication with applicants and internal staff.

Qualifications

  • Preferred Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, or related field.
  • 1-3 years of experience in admissions, case management, or a related human services field.
  • 2-3 years of experience delivering services to persons with disabilities.
  • Proficiency in all Microsoft Office applications.
  • Qualification standards as found in 245D, Adult Foster Care, Background Studies of Individuals Affiliated with Programs Licensed by DHS.

Responsibilities

  • Completes all intake paperwork, including obtaining signatures from all team members.
  • Creates individual profiles and folders in Therap and Teams, and uploads all corresponding paperwork into both systems.
  • Uploads referrals into SharePoint and maintains the referral waitlist.
  • Attends all pre-intake meetings.
  • Assists with responding to all placement inquiries in a timely manner and may provide tours to prospective individuals.
  • Demonstrates a working knowledge of company policies and procedures related to services.
  • Demonstrates a working knowledge of all site’s layout and staffing patterns to ensure appropriate placements.
  • Provides professional, enthusiastic, and efficient support to the Services Team and other Company leaders as directed.
  • Performs a variety of administrative tasks including filing, scanning, faxing, etc.
  • Other duties and responsibilities as assigned.

Job description

The primary purpose of this position is to assist with residential and crisis services admissions and coordinating the intake process. This position reports to the Admissions Director. The position is classified as hourly,non-exempt.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

JOB REQUIREMENTS:
  • Ability to move intermittently throughout the workday.
  • Demonstrate skills in reading, writing, and speaking the English language fluently.

  • Implement risk management measure to avoid and/or ameliorate risky practices, non-practices/incorrect practices or violations of any sort.
