Job summary

  • Proficiency in Microsoft Office applications.

Responsibilities

  • Completes intake paperwork and obtains signatures from team members.
  • Creates profiles and folders in Therap and Teams and uploads documents.
  • Uploads referrals into SharePoint and maintains referral waitlist.
  • Attends pre-intake meetings and answers placement inquiries.
  • Provides support to Services Team and Company leaders as directed.
  • Performs administrative tasks such as filing, scanning, and faxing.

The primary purpose of this position is to assist with residential and crisis services admissions and coordinating the intake process. This includes managing the referral and waitlist tracking in SharePoint, completing intake paperwork, maintaining individual profiles and records in Therap and Teams , and attending intake meetings .

This position reports to the Admissions Director. The position is classified as hourly , non-exempt.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Completes all intake paperwork, including obtaining signatures from all team members.
  • Creates individual profiles and folders in Therap and Teams, and uploads all corresponding paperwork into both systems.
  • Uploads referrals into SharePoint and maintains the referral waitlist.
  • Attends all pre-intake meetings.
  • Assists with responding to all placement inquires in a timely manner and may provide tours to prospective individuals.
  • Demonstrates a working knowledge of company policies and procedures related to services.
  • Demonstrates a working knowledge of all site’s layout and staffing patterns to ensure appropriate placements.
  • Provides professional, enthusiastic, and efficient support to the Services Team and other Company leaders as directed.
  • Performs a variety of administrative tasks including filing, scanning, faxing, etc.
  • Other duties and responsibilities as assigned.

JOB REQUIREMENTS:

  • Ability to move intermittently throughout the workday.
  • Demonstrate skills in reading, writing, and speaking the English language fluently.

  • Implement risk management measure to avoid and/or ameliorate risky practices, non-practices/incorrect practices or violations of any sort.

  • Ability to maintain a high level of confidentiality and professionalism is required in order to make administrative and procedural decisions and judgments on sensitive and issues.
  • Must be adaptable, have self-motivation, and strong follow through skills
  • Possess cell phone or maintain adequate phone access in order to allow for employee contact.

QUALIFICATIONS:

  • Preferred Bachelor’s Degree in Social Work , Human Services, Psychology, Sociology, or related field.
  • 1-3 years of experience in admissions, case management, or a related human services field.
  • 2-3 years of experience delivering services to persons with disabilities.
  • Qualification standards as found in 245D, Adult Foster Care, Background Studies of Individuals Affiliated with Programs Licensed by DHS.
  • Proficiency in all Microsoft office applications.

WORKING CONDITIONS:

  • Work in a variety of settings throughout the community.
  • Sit, stand, walk, bend, lift, and move intermittently during working hours.
