ADMISSIONS COORDINATOR

Summary

Coordinates admissions to LifeBridge Health Post Acute Facilities.

Responsibilities

Coordinates all aspects for a potential admission to LifeBridge Health Post Acute Facilities. Ensures notification of the Admissions Liaison of referrals to be evaluated for medical eligibility. Ensures financial eligibility and a payment mechanism for all admissions to LBH post acute facilities following financial process. Ensures the collection and sending of appropriate clinical documents to the units. Meets with families to discuss admission possibilities. Conduct Tours of the facility when available. Works with Unit staff, Clinical Leaders, Physicians and Patient Accounting to ensure clinical and financial appropriateness. Verify insurance and ability to present clinical cases to secure authorizations for LTACH and LTC to insurance companies. Report and coordinate bed management regarding placement and availability. Notify staff of all new admissions and necessary related information. Knowledge of EMR systems and able to navigate throughout. Maintain and update census and tentative admissions list. Performs clerical duties as directed. Maintains current status on open and closed referrals. Coordinates all patient/resident admissions to the facility, and completes all paperwork necessary, prior to and at the time of admission.
