Job summary

Signature HealthCARE seeks candidates to coordinate admissions and transitional care, liaising with care teams to meet census and bed management targets. What you Need to make a Difference

• Minimum of one (1) year experience in long-term care or related industry.
• Local travel with your personal vehicle is required.
• Must understand long-term care business, including census and quality mix goals and referral expectations.
• Demonstrated intermediate to advanced skills in Microsoft Office Suite.
• Must have highest level of professionalism with the ability to maintain confidentiality.
