Job summary

Signature HealthCARE LLC is seeking an admissions-focused professional to coordinate communications between care teams and families. The role requires understanding census metrics and bed management, plus compliance with federal and state regulations affecting referrals and reimbursement.

You will travel locally using your own vehicle to meet potential residents and families, ensuring smooth transitions and high-quality outcomes in a long-term care setting.

Qualifications

• Communicating admissions and transitional care needs with the appropriate stakeholders involved in care.
• Understand census, quality mix, and bed management needs of the building.
• Maintian a working knowledge of Federal and State regulations, referral processes and reimbursement (Managed Care, Medicare, and Medicaid).
• Spend time in hospitals meetings with potential residents and families.

What you Need to make a Difference

• Minimum of one (1) year experience in long-term care or related industry.
• Local travel with your personal vehicle is required.
• Must understand long-term care business, including census and quality mix goals and referral expectations.
• Demonstrated intermediate to advanced skills in Microsoft Office Suite.
• Must have highest level of professionalism with the ability to maintain confidentiality.
