Job summary

Signature HealthCARE is seeking an Admissions Coordinator to support long-term care facilities. The role involves coordinating admissions and transitional care with care teams, understanding census and bed management, and staying current on regulatory and referral processes to optimize reimbursements.

The position requires travel within local facilities, strong Microsoft Office skills, professionalism, and confidentiality.

Qualifications

• Minimum of one year experience in long-term care or related industry.
• Local travel with your personal vehicle is required.
• Must understand long-term care business, including census and quality mix goals and referral expectations.
• Demonstrated highest level of professionalism with the ability to maintain confidentiality.
• Proficient in Microsoft Office Suite.

Responsibilities

• Communicate admissions and transitional care needs with the appropriate stakeholders involved in care.
• Understand census, quality mix, and bed management needs of the building.
• Maintain knowledge of Federal and State regulations, referral processes and reimbursement (Managed Care, Medicare, and Medicaid).
• Spend time in hospital meetings with potential residents and families.
