Job summary

Signature HealthCARE TN, Memphis is seeking a motivated team member to support admissions and transitional care needs across our long-term care facilities. You will coordinate with care teams and families to ensure smooth transitions.

• Must understand long-term care business, including census and quality mix goals and referral expectations.
• Demonstrated intermediate to advanced skills in Microsoft Office Suite.
• Must have highest level of professionalism with the ability to maintain confidentiality.

• Understand census, quality mix, and bed management needs of the building.
• Maintian a working knowledge of Federal and State regulations, referral processes and reimbursement.

• Communicating admissions and transitional care needs with the appropriate stakeholders involved in care.
• Understand census, quality mix, and bed management needs of the building.
• Maintian a working knowledge of Federal and State regulations, referral processes and reimbursement (Managed Care, Medicare, and Medicaid).
• Spend time in hospitals meetings with potential residents and families.

What you Need to make a Difference

• Minimum of one (1) year experience in long-term care or related industry.
• Local travel with your personal vehicle is required.
• Must understand long-term care business, including census and quality mix goals and referral expectations.
• Demonstrated intermediate to advanced skills in Microsoft Office Suite.
• Must have highest level of professionalism with the ability to maintain confidentiality.
