Job summary

Chapters Health System is seeking an Admission RN to assess patients in their residence or hospital settings and transition care to Care Navigation. You will serve as a company ambassador, ensuring smooth patient admissions and seamless discharge planning.

The role requires an RN license, at least 1 year of nursing experience (hospice/hospital preferred), and EMR proficiency. Strong communication, teamwork, and the ability to handle multiple priorities are essential.

Qualifications

• Must be a licensed RN in the state of employment.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment and time-management skills.
• Excellent English communication; Spanish bilingual a plus.
• Valid driver’s license and reliable transportation.
• Ability to work in a team and handle multiple priorities.

Responsibilities

• Complete clinical admissions for all Chapters Health System services.
• Explain services to patients and families; address questions with empathy.
• Initiate Plan of Care and coordinate with physicians.
• Perform nursing interventions to prevent complications and maximize comfort.
• Maintain timely documentation and SBAR communications.
• Coordinate with Care Navigation and support discharge planning.
• Adhere to policies, safety, and regulatory requirements.
