Job summary

Tri-State Village Nursing and Rehabilitation Center in Lansing, IL is seeking a compassionate Admission Coordinator to guide new residents and families through the enrollment process, greet and orient them, and coordinate with healthcare providers.

You will assess residents, develop care plans aligned with center goals, maintain accurate records, and collaborate with the interdisciplinary team while upholding HIPAA regulations.

• Collaborated with our interdisciplinary team to ensure that residents receive comprehensive care and support
• Maintained accurate and up-to-date records and reports, including admission and discharge data
• Participated in quality improvement initiatives to enhance the overall resident experience
• Maintained confidentiality and adhered to all HIPAA regulations
