Job summary

Dove Healthcare – West Eau Claire is seeking an admissions coordinator to join our Eau Claire – West location at 1405 Truax Blvd, Eau Claire, WI 54703. This full-time role serves multiple Dove Healthcare locations and supports admissions from hospital discharge planners and other referral sources.

You will assess and coordinate admissions, confirm medical and financial information, build relationships with referral sources, and help ensure smooth patient transitions and continuity of care.

Qualifications

• Professional health care experience is required.
• Ability to multitask in a fast-paced environment.
• Proficiency with Microsoft Office and healthcare software.
• Experience with EPIC/Care Anywhere, Point Click Care is helpful.
• Strong organizational and time-management skills.
• Excellent written and verbal communication abilities.

Schedule Details:

• Full-time position (40 hours / week)
• Will be based at our Eau Claire – West location serving multiple Dove Healthcare locations

Required Qualifications

• Professional health care experience required (social worker, nurse CMA, CNA, pharmacy tech, PTA, COTA, etc.)
• Ability to multitask, juggle multiple priorities and work in a fast-paced environment
• Proficiency in relevant software tools / applications such as Microsoft.
• Experience with EPIC/Care Anywhere, Point Click Care (CRM) is helpful.
• Detailed oriented with strong organizational and time management skills
• Strong problem-solving abilities
• Proficiency in written and verbal communications

Working as an admissions coordinator, you will be responsible for:

• Coordinating admissions with hospital discharge planners and other referral sources
• Assessing, coordinating and ensuring a smooth admission transition
• Confirms medical and financial informationneeded to ensure a smooth admission transition
• Develops and maintains excellent networking skills and relationship building with referral sources
• Ensuring patient continuity of care
