Qualifications

  • Bachelor's degree.
  • 7+ years of progressive healthcare management experience, with 5+ years in Home Care leadership.
  • Strong knowledge of Home Care Medicare Conditions of Participation and JCAHO accreditation standards.
  • State of Ohio RN or Therapy (PT, OT or SLP) license.
  • Current BLS certification (AHA or Red Cross).

Responsibilities

  • Lead with Director of Nursing to manage clinical operations and business functions.
  • Develop and monitor annual operating and capital budgets.
  • Foster a quality, accountable, collaborative caregiver work environment.
  • Oversee financial, operational, and quality outcomes using available resources.
  • Coach, mentor, and develop caregivers.
  • Develop and implement homecare policies and center-wide procedures.
  • Participate in committees and lead leadership meetings and town halls.
  • Ensure compliance with regulatory standards (JCAHO, CMS, ODH) and accreditation.
  • Collaborate across Center for Connected Care to improve patient care.
  • Represent Homecare within the health system and to external agencies.
  • Foster teamwork and continuous improvement with data reporting to stakeholders.

The Homecare Administrator provides strategic and operational leadership for Cleveland Clinic Homecare, supporting approximately 2,000 patients across 19 counties with a workforce of more than 300 caregivers. This role is responsible for the overall performance of the homecare service line, including operations, regulatory compliance, quality outcomes, financial performance, workforce engagement and retention, growth initiatives and strategic planning. The Administrator collaborates closely with Center for Connected Care business line leaders and other key stakeholders to advance strategic initiatives, strengthen care transitions, and ensure high-quality, coordinated care across the continuum

A caregiver in this role will work Monday through Friday from 8:00am to 5:00pm and will be on-call for emergencies.

A caregiver who excels in this role will:

  • Work closely with the Director of Nursing, direct clinical management, business operations and all supporting departments.
  • Prepare, develop and implement annual operating and capital budgets; monitors ongoing financial performance and makes corrections to plan if necessary.
  • Create and support a work environment that fosters quality, accountability, collaboration and caregiver satisfaction.
  • Manage and control financial, operational and quality outcomes of agency by effective use of resources, both human and material.
  • Ensure development of all caregivers through effective coaching, counseling and mentoring, leading by example.
  • Direct the development and implementation of homecare policies and procedures, and contribute to center-wide policies, procedures and emergency planning.
  • Participate in applicable committees. Guide homecare leadership meetings and town halls.
  • Oversee agency service delivery that is in compliance with regulatory/accreditation standards such as JCAHO, CMS, and ODH, and emulates “best practices”; ensures agency performance that will sustain applicable agency certification/accreditation.
  • Develop and maintain a home health compliance plan and OIG work plan in accordance with the Center for Connected Care compliance program.
  • Delegate adequate authority to achieve assigned responsibilities and ensures mechanisms for accountability.
  • Support and contribute to the development of new programs and services, as relevant.
  • Collaborate with the other Center for Connected Care business lines and departments within the health system to deliver clinical services that are in concert with the mission, vision, and values of the organization, yielding positive clinical outcomes and high customer satisfaction.
  • Support the development of evidence-based practice through participation in and contribution to the literature, investigational studies, etc.
  • Be responsible for building and maintaining relationships with referral sources and meeting growth targets of the department.
  • Represent Cleveland Clinic Homecare throughout the health system and to outside agencies and organizations.
  • Foster a team approach to patient care and encourage collaboration amongst teams.
  • Promote and support a continuous improvement environment, which incorporates both quantitative and qualitative analysis in order to accomplish effective problem solving and planning; provides outcome reports to key stakeholders at appropriate intervals.
  • Must be able to perform all direct care responsibilities as a licensed clinician, as appropriate based on licensure: RN, PT, OT, or SLP
Minimum qualifications for the ideal future caregiver include:
  • Bachelor's degree
  • A minimum of 7 years of progressive healthcare management experience, with a minimum of 5 years of Home Care experience in a leadership position required
  • Strong knowledge of the Home Care Medicare Conditions of Participation and JCAHO accreditation standards
  • State of Ohio RN or Therapy (Physical Therapist (PT), Occupational Therapist (OT) or Speech Therapy (ST)) license
  • Current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross
Preferred qualifications for the ideal future caregiver include:
  • Master's degree
  • Nursing or physical therapy experience
  • Certification in healthcare/ homecare administration
Physical Requirements:
  • Requires sitting, standing and walking for extended periods of time. Requires manual dexterity to perform work on a PC.
  • Requires good visual acuity through normal or corrected vision and must be able to hear typical conversation levels through normal or corrected hearing.
Personal Protective Equipment:
  • Follows standard precautions using personal protective equipment as required.
Pay Range
  • Minimum Annual Salary: $112,110.00
  • Maximum Annual Salary: $170,955.00
