Job summary

Geisinger is seeking a patient support associate to monitor and assist patients and facilitate recovery-oriented activities in a clinical setting. The role emphasizes attentive patient engagement, documenting care, and collaborating with care teams to ensure safety and treatment adherence.

You will provide evening and weekend support, assist with groups, collect bio-psycho-social data, orient new patients, and communicate important events to staff while maintaining compliance with facility rules

Qualifications

• Experience supporting patients in a clinical or recovery-oriented setting.
• Ability to assist with groups and documentation.
• Skilled at collecting bio-psycho-social data.
• Able to communicate effectively with patients, families and staff.

Responsibilities

• Facilitates adjunct groups/meetings and performs all associated duties, including documentation.
• Provides support to patients during evenings and weekends, and when needed, helps to de-escalate emotional reactions.
• Collects appropriate data on bio-psycho-socials.
• Completes initial family contacts and provides information on family services and documents appropriately; facilitates sign-up for Family Day Program and monitors family visitations.
• Provides new patients with general orientation to the facility, including introduction to the facility rules and regulations.
• Monitors patients during free time and their preparation for bedtimes, and conducts bed and room checks.
• Communicates significant events via the communication log, patient charts and verbally to key staff.
