Job summary

CleanSlate Centers in Phoenix, AZ is seeking a licensed behavioral health professional to guide patients through recovery using evidence‑based interventions and person‑centered treatment planning. You will collaborate with a multidisciplinary team to ensure holistic support addressing clinical, emotional, social, and behavioral needs.

Qualifications

• Master’s degree required.
• Current and unrestricted state licensure to practice independently as a behavioral health professional (e.g., LCSW, LPC, LMFT).
• Experience in substance use disorder treatment preferred.
• Proficiency in evidence‑based counseling practices and treatment planning.
• Strong communication skills and rapport-building abilities.
• Familiarity with ethical, regulatory, and confidentiality standards in behavioral health.
• Ability to work independently while collaborating with a treatment team.

Responsibilities

• Conduct biopsychosocial assessments and create individualized treatment plans.
• Facilitate individual and group counseling sessions focused on recovery and behavior change.
• Maintain thorough documentation of treatment progress.
• Modify treatment plans based on clinical feedback.
• Respond to crises and provide immediate support or intervention.
• Collaborate with medical providers, case managers, and community partners.
• Support aftercare planning and referrals.

Education
