Qualifications

• Master's degree in Social Work, Counseling, or related field.
• LMSW/LPC-A/LMFT-A or CADC licensure preferred.
• Valid driver's license required.
• Computer proficiency required for documentation.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct comprehensive bio-psychosocial assessments and care plans.
• Coordinate level of care and referrals with the interdisciplinary team.
• Deliver direct patient care, counseling, and support to patients and families.
• Provide education on disease prevention, treatment options, and resources.
• Participate in team meetings, supervision, and program development.
• Maintain timely patient documentation for regulatory and third-party needs.
• Support hospital programming and strive for optimal patient experience.

Education

Master's Degree in Social Work, or Counseling, or Marriage and Family Therapy or Related Human Service Field

LMSW, LPC-A, LMFT-A, or CADC

Valid Driver's License
