Job summary

Hartford HealthCare’s Rushford Center is seeking a licensed or eligible social worker to provide evidence-based clinical services across the continuum of care in Middletown, CT. You will work with interdisciplinary teams to assess needs, develop care plans, and coordinate referrals with a focus on compassionate, culturally competent care.

Qualifications

• Master's Degree in Social Work, or Counseling, or related Human Service field.
• Licensure such as LCSW, LPC, LMFT or LADC or eligibility to obtain.
• Experience in a healthcare setting.
• Excellent communication and cultural competence.
• Ability to assess problems, determine solutions and make timely decisions.
• Supervision and team collaboration in patient assessment and interventions.

Responsibilities

• Conduct comprehensive bio-psychosocial assessment and establish a care plan.
• Collaborate with the interdisciplinary team to determine level of care and coordinate care.
• Use therapeutic interventions as per individualized care plans.
• Monitor progress and revise plans to meet patient/family needs.
• Educate patients and families on disease management and treatment adherence.
• Provide direct social work interventions across patient populations.
• Document patient information timely and accurately per regulations.
• Supervise social work students and participate in team activities.
• Support programming and initiatives to meet clinical and strategic needs.
