Job summary

Affordable Rent seeks an Addiction Specialist Coordinator to support individuals with Substance Use Disorders by coordinating comprehensive care and engaging MAT services. You will reinforce connections between hospital discharges, outpatient care, home health, and case management to promote continuity and stabilization.

Embedded within the SIPPRA team, you will help launch a Contingency Management program in supportive housing, develop care plans, monitor progress, and maintain documentation in

Qualifications

• High School Diploma or GED required.
• Bachelor's degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Combination of education and experience may be substituted.
• Colorado driver’s license within 30 days of hire (SSH C may waive depending on needs).

Responsibilities

• Coordinate comprehensive care for clients across behavioral health, medical, psychiatric, and case management services.
• Facilitate referrals and transitions from hospitals or detox programs.
• Support MAT adherence and follow-up with care teams.
• Implement and monitor Contingency Management initiatives.
• Provide client-centered care management focusing on stabilization and housing retention.
• Ensure confidentiality and high-quality documentation in the EHR.
• Collaborate across disciplines and community partners to improve access to services.
• Uphold professional standards and participate in team learning and development.
