Job summary

SSTAR is seeking an Entry Level Nurse Case Manager for the MAT Program to assess and triage patients, develop care plans, and coordinate enrollment into MAT services. You will work with the MAT team to support patients through induction and maintenance while collaborating with PCPs and behavioral health specialists.

The role requires MA RN licensure, CPR, and experience with addiction treatment or mental health care.

Qualifications

• RN License valid in Massachusetts.
• Current CPR Certification required.
• Experience caring for patients with mental illness or SUD.
• Ability to work autonomously and lead as needed.
• Knowledge of addiction treatment and MAT processes.

Responsibilities

• Assess and triage patients; develop a care plan for MAT program.
• Coordinate enrollment into MAT services with the MAT team.
• Provide direct patient care and counseling for substance use disorders.
• Facilitate referrals with PCPs, behavioral health providers, and other team members.
• Support patient care across OBAT treatment stages: induction, stabilization, maintenance.
