Job summary

Colorado Coalition for the Homeless is seeking an Addiction Specialist Coordinator to coordinate comprehensive care for clients with Substance Use Disorders, linking hospital, outpatient, home health, and case management for stability and continuity.

The role supports MAT, CM initiatives, and care planning, while maintaining detailed documentation and collaboration with multidisciplinary teams in a supportive housing setting.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Combination of education and experience may be substituted on a case-by-case basis.
• Colorado driver’s license within 30 days of hire.

Responsibilities

• Coordinate comprehensive care for clients with SUD across behavioral health, medical, and case management services.
• Facilitate access to care by coordinating referrals and transitions from hospitals or detox programs.
• Support MAT adherence and engagement with care teams for timely follow-up.
• Implement Contingency Management initiatives and document outcomes.
• Maintain accurate EHR documentation to ensure continuity and compliance.

Education
