Baystate Health is seeking an Addiction Consultation Coordinator for the Psychiatry Consultation Service in Springfield, MA. You will support inpatient patients with substance use needs, linking them to IO P and PHP programs and coordinating with social work, case management and the care team during hospital stays and transitions.

Qualifications

• 1 year of experience in a hospital or medical setting is preferred.
• 2 years working with individuals with substance use disorders preferred.
• Minimum 2 years in multidisciplinary behavioral health or medical social work/care management.

Responsibilities

• Visit inpatient patients and assist across medical floors as needed.
• Connect patients with addiction treatment services and community resources.
• Collaborate with Social Work, Case Management and care team for transitions.
• Document interactions and care coordination activities in the medical record.
• Serve as primary Addiction Coordinator to ensure timely connections to services.
