Job summary

Kaiser Permanente in Atlanta, GA is seeking an Acute Transitional Case Manager to coordinate care for members with complex conditions, guiding discharge from acute care or skilled facilities to home or lower levels of care. You’ll work with hospital physicians, QRM staff, social workers, and other providers to ensure safe, timely discharge and post-acute follow-up.

Requirements include 3+ years in acute discharge planning or case management, a GA RN/LCSW/LMSW license, and willingness to travel

Qualifications

• Minimum three (3) years acute hospital discharge planning or prior experience in case management as an RN.
• High School Diploma or GED required.
• GA RN/LCSW/LMSW license or active licensure in Georgia.

Responsibilities

• Coordinate all transitional case management activities outlined above.
• Conduct timely reviews and refer cases per policy; evaluate continuity of care.
• Perform telephonic assessment of member's physical, psychosocial, environmental, and health status.
• Develop individual care plans with KP Hospitalists and practitioners with timelines.
• Attend care conferences with QRM physicians and IPCC team to discuss course and barriers.
• Implement plan of care and interventions; coordinate post-acute services and follow-ups.
• Document interactions and interventions per departmental guidelines.
• Coordinate and participate in complex case management conferences; monitor care quality.
