Job summary

Kaiser Permanente is seeking an Acute Transitional Case Manager to coordinate care for members with complex conditions and ensure smooth discharge from hospital or post-acute settings. You will partner with hospitalists, case managers, social workers, and providers to create effective discharge plans and arrange post-acute services.

The role requires experience in discharge planning, appropriate licensure in Georgia, and willingness to travel within the Atlanta metro area.

Qualifications

• Minimum three (3) years acute hospital discharge planning or prior experience in case management as an RN.
• High School Diploma or GED required.
• GA Registered Nurse license or GA LCSW or GA LMSW.
• Complex Case Management experience.
• Willingness to travel within the Atlanta metro area.

Responsibilities

• Coordinate care for identified members with complex medical conditions.
• Develop safe discharge plans from acute inpatient or skilled facilities.
• Communicate plan of care to primary/specialist providers and post-acute services.
• Attend care conferences and collaborate with physicians and care teams.
• Provide telephonic outreach post-discharge and monitor plan adherence.
• Document all case management interactions per guidelines.
• Assist in development and maintenance of QRM policies and protocols.
