Mercy is seeking an RN for our Acute Rehab unit. You will deliver direct nursing care, perform assessments, administer medications, and develop rehab-focused care plans.

You will coordinate with physicians, therapists and the care team to optimize patient outcomes while educating patients and families on mobility, safety, and discharge needs. Mercy emphasizes compassion, diversity, and professional growth.

Qualifications

• Develop, implement, and update rehab-focused care plans.
• Coordinate with physicians, therapists and the interdisciplinary team.
• Document in EHR and monitor patient progress.
• Educate patients and families on mobility, discharge needs.

Responsibilities

• Deliver direct nursing care to acute rehab patients with assessments and meds.
• Develop and update individualized rehab plans.
• Collaborate with physicians and therapists to optimize outcomes.
• Monitor progress, document changes in EHR, communicate promptly.
• Educate patients and families on recovery, mobility and discharge plans.
• Maintain safety, infection control and quality standards on unit.
• Participate in team huddles and performance improvement initiatives.
