Job summary

Advocate Health seeks a part-time Physical Therapist at Advocate Condell Medical Center – Acute Care Rehabilitation in Milwaukee, WI. You will interpret referrals, evaluate patients, and develop an individualized plan of care to improve mobility and function.

You will discharge patients with home programs, educate families, and document outcomes per policy. The role requires weekend, late shifts, and one holiday annually, with a pay range of $41.10–$61.65 per hour and comprehensive benefits.

Qualifications

• Doctorate degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state of practice; BLS required.
• No prior experience required; strong communication and organization needed.
• Intermediate computer skills and ability to complete documentation.

Responsibilities

• Evaluate musculoskeletal, neuromuscular, cardiopulmonary and integumentary systems as appropriate.
• Develop and implement a plan of care with functional goals and adjust as needed.
• Discharge planning, home programs, patient/family teaching and community referrals.
• Document care, scheduling, billing and adherence to policies and timelines.
• Maintain safety, infection control and equipment use; participate in safety initiatives.
• Assist with supervision of staff and ongoing education as requested.
