Job summary

Fort HealthCare in Fort Atkinson, WI is seeking an Acute Care Physical Therapist for weekend rotation. Responsibilities include evaluating and treating patients with musculoskeletal and/or neurological impairments on the hospital rehab team, with one weekend shift per month and occasional part-time scheduling.

You will document patient status, progress, and discharge notes, communicate with physicians and interdisciplinary team members, and participate in delivering compassionate, high-quality

Qualifications

• Licensed physical therapist in Wisconsin (WI PT license).
• CPR/BLS certification required.
• Ability to work weekends and part-time schedule.

Responsibilities

• Performs physical therapy evaluations and develops a plan of care that is appropriate for the diagnosis and age of patient.
• Performs physical therapy patient treatment as per the plan of care and modifies treatment plan based on ongoing assessment of patient condition.
• Demonstrates an understanding of the operation and indications for commonly used modalities and equipment.
• Documents evaluations, daily notes, progress notes, and discharge notes in a timely manner; therapy charges are supported by this documentation.
• Communicates and documents necessary information appropriately with patients, families, physicians, and interdisciplinary team members.
