Job summary

BayCare Health System in Winter Haven, FL is seeking an Acute Physical Therapist for a Pool/PRN position to join a hospital-based, multi-disciplinary team. Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Deliver evidence-based interventions to improve mobility, strength, balance and functional independence.
• Collaborate with physicians, nurses, case managers and other therapists to coordinate care.
• Educate patients and families on exercises, safety and post-discharge recommendations.
• Document all patient care activities accurately in the EMR and participate in quality improvement initiatives.

Acute Physical Therapist Pool/PRN Responsibilities:

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.

• Deliver evidence-based interventions to improve mobility, strength, balance, and functional independence.

• Collaborate with physicians, nurses, case managers, and other therapists to coordinate care.

• Educate patients and families on exercises, safety, and post-discharge recommendations.

• Document all patient care activities accurately and timely in the electronic medical record.

• Participate in team meetings and contribute to quality improvement initiatives.

Requirements:

• Required BLS (Basic Life Support)

• Required PT (Physical Therapy)

• Or PT (Physical Therapy) Eligible to obtain

• Or PT (Physical Therapy) Temporary and licensure within 6 months

• Required Bachelor's Physical Therapy

• Preferred 1 year Physical Therapist
