Job summary

Qualifications

• Bachelors in Physical Therapy from an accredited university (APTA approved).
• Masters in or Doctorate in Physical Therapy from an accredited university approved by the APTA.
• Arizona State Board of Physical Therapy license or PT Compact License in good standing.
• Basic Life Support (BLS) certification required.

Responsibilities

• Implements an individualized treatment plan using evidence-based therapy techniques.
• Provides verbal and written instructions to patients and families as needed.
• Evaluates patients and develops a comprehensive plan of care with measurable goals and discharge plan.
• Collaborates with the interdisciplinary team and discusses patient status with relevant personnel.
• Maintains accurate and current documentation in the patient’s medical record.
