Responsibilities

• Clinical review, coordination and management of acute care hospital patients with high complexity.
• Perform comprehensive initial assessments of medical, psychosocial, and discharge needs.
• Daily utilization review to determine medical necessity and level of care.
• Coordinate with physicians, hospital staff and care teams for timely transfers and discharge planning.
• Educate patients and families on advance directives and care planning.
• Travel to Southern California facilities approximately one day per week for on-site case management.
