Job summary

Heritage Provider Network, Inc. in Bakersfield, CA is seeking an LVN or RN for Acute Tertiary Case Management. The role covers high-acuity, complex cases and requires travel to tertiary facilities in Southern California about one day per week.

You will conduct initial assessments, lead utilization reviews, and coordinate discharge planning across care settings to promote safe, efficient outcomes.

Qualifications

• Graduate from an LVN or accredited RN program.
• Current California LVN or RN license.
• Current BLS certification.
• Minimum of three (3) years of acute care clinical nursing experience.
• Minimum of two (2) years of Utilization Review and/or Acute Case Management experience preferred.
• Experience with medically complex patients in acute/tertiary care settings preferred.
• Knowledge of discharge planning and care transitions preferred.

Responsibilities

• Provides case management coverage for acute care patients on a rotational schedule.
• Performs comprehensive initial patient assessment of medical, psychosocial, functional, and discharge needs.
• Conducts daily utilization review to determine medical necessity and level of care.
• Coordinates with hospital staff to expedite diagnostics, procedures, and transfers.
• Maintains ongoing communication with physicians, specialists, and care teams.
• Coordinates comprehensive discharge planning across multiple care settings.
• Educates patients and families regarding advance directives and care planning.
• Travels to Southern California approximately one day per week for on-site activities.
