Job summary

High Desert Medical Group is seeking a full time Acute Case Manager to evaluate inpatient records and coordinate discharge planning in acute, skilled, and ER settings. Responsibilities include SI/IS criteria reviews, hospitalist communication, and on-call coverage.

Required: LVN/RN with current license, CPR certification, and at least 2 years clinical experience. Strong written and verbal communication skills and ability to multitask under pressure are essential.

Qualifications

• High School diploma or equivalent.
• Graduate from an accredited LVN or RN Nursing Program, with current LVN or RN licens(e).
• Current CPR certification.
• Minimum of 2 years clinical experience; Case Management experience preferred.
• Excellent written and verbal communication skills.
• Ability to handle and prioritize tasks to meet deadlines and productivity goals.
• Ability to handle confidential information.
• Ability to work in a multi-task, high-demand environment.

Responsibilities

• Perform inpatient reviews daily for HDMG in acute settings, skilled settings, and ER.
• Review medical records for SI/IS criteria.
• Communicate with hospitalists, attending physicians, and specialists about utilization.
• Coordinate with care coordination regarding discharge planning.
• Identify and initiate discharge planning needs with case management staff.
• Assist hospitalist and physician to identify continuum of care needs.
• Facilitate discharge plans.
• Attend Utilization Meetings and acute rounds as needed.
• Participate in the on-call rotation for Case Management.
• Perform other duties as assigned.
