Qualifications

• Bachelor’s degree in behavioral health or related field required.
• CPR and de‑escalation and restraint certification required.
• Prior behavioral health case management experience preferred.

Responsibilities

• Develops the service plan with the treatment team.
• Coordinates discharge planning and aftercare options.
• Maintains comprehensive clinical records and coordinates with referrals.
• Facilitates connections to psychiatric and community services for housing, employment, etc.
