Job summary

Saint Joseph Medical Center - Joliet is seeking a full-time Social Worker to provide emotional support and counseling for Pediatrics to Adults, while coordinating discharge planning as part of a multidisciplinary team.

Qualifications

• Master’s Degree in Social Work or related field (required).
• Minimum one year hospital or outpatient experience with emphasis in mental health care.
• Current licensure/registration as required by state within 3 months of hire.

Responsibilities

• Provide emotional support, crisis intervention and counseling across age groups.
• Perform psychosocial assessments and assist in developing and re-evaluating treatment plans.
• Coordinate discharge planning and work as part of a multidisciplinary team.
