Job summary

Prime Healthcare through Saint Mary of Nazareth Hospital in Chicago is seeking a Social Worker to provide emotional support, crisis intervention and counseling, along with information and referrals to patients and families dealing with illness, disability or trauma. Qualifications

• Bachelor’s degree in Social Work, Marriage and Family Therapy, or related field.
• Current registration or internship with the Board, as appropriate to education/licensing requirements.
• Minimum one year inpatient or outpatient behavioral health experience preferred.

Responsibilities

• Provide emotional support, crisis intervention, counseling and information/referral to patients and families.
• Coordinate discharge planning and complete psychosocial assessments; work as part of a multidisciplinary team.
• Complete necessary documentation and maintain clinical records for each client during length of stay.

Education

Bachelor’s degree Social Work, Marriage and Family Therapy or other related required.

Current registration or intern with Board., as appropriate to education and licensing requirements.

Minimum one year inpatient or outpatient behavioral health experience preferred.
