Ann & Robert H. Lurie Children's Hospital of Chicago is seeking a social worker to coordinate care for children and families, coordinating referrals, and supporting discharge planning within a multidisciplinary team.

The role involves addressing psychosocial, cultural, and spiritual needs, intervening in crises, and documenting thoroughly to support optimal patient outcomes. Strong communication and teamwork are essential.

Qualifications

• Master's degree in social work from an accredited program.
• Current Illinois State Social Work licensure (LSW) required; LCSW preferred.
• Experience working with children and families in health care, child welfare, or mental health settings.

Responsibilities

• Coordinates service delivery with families and team members across the continuum.
• Makes referrals to other team members and community resources.
• Collaborates with families to utilize community service linkages.
• Meet with the inpatient case manager daily to review census and identify discharge barriers.
• Address child/family psychosocial, developmental, cultural, and spiritual needs.
• Provide timely intervention in emergencies or crisis situations.
• Document thoroughly according to professional standards and communicate with health care team.
• Assist case managers with discharge planning and community linkages for complex needs.
• Maintain professional boundaries and participate in interdisciplinary rounds.
• Educate team on how psychosocial issues impact treatment and coping.
