Job summary

• Develop and update individualized care plans and document outcomes
• Assess patient needs upon admission and throughout hospitalization
• Ensure accurate documentation of care, pain management, and patient responses
• Coordinate discharge planning and communication with patient/family
• Promote patient/family centered care and safety throughout care episodes
• Maintain compliance with regulatory, privacy, and security standards
