Job summary

Qualifications

• Must have nursing licensure and be able to provide evidence of BLS/ACLS where required.
• Must perform initial assessments and develop care plans.
• Proficient in patient education, discharge planning and reporting.

Responsibilities

• Conducts an initial assessment upon patient admission.
• Develops outcome-based care plans for patients.
• Coordinates care with physicians, case manager and other staff.
• Documents care accurately and communicates changes to the team.
• Manages telemetry and cardiac monitoring when applicable.
• Ensures medication safety and adheres to policies.
• Maintains safe, clean, and patient-centered care environment.
