Downtown Evansville Inc. is seeking a Registered Nurse - Acute Care Case Manager to support patient journeys through the healthcare system. The role emphasizes advocacy, coordination, and safe transitions across the continuum of care.

As an on-site, part-time RN, you will collaborate with physicians, nurses, therapists, social workers, patients, and families to remove barriers and ensure timely, appropriate care. Indiana/Kentucky RN licensure (or compact) is required.

Qualifications

• Active Indiana/Kentucky RN license or compact RN license.
• Minimum of 2 years of acute care RN experience.
• PRN availability for day shifts.

Responsibilities

• Drive patient journeys through the healthcare system, coordinating services and creating safe transitions.
• Collaborate with physicians, nurses, therapists, social workers, patients, and families to remove barriers.
• Advocate for patients to improve outcomes and quality of care across the continuum.
